Gallatin HS – Athlete Pre-Participation Information
Dear Parents and/or Guardians,
Below is information regarding mandatory athlete pre-participation paperwork including sport physicals and the online ATS athlete portal. This paperwork is a combination of forms required by Sumner County Schools, Tennessee Secondary School Athletic Association (TSSAA), BodyGuard Sports Medicine, and Gallatin High School (GHS). BodyGuard is the official sports medicine provider for GHS and provides a certified athletic trainer for the medical care of student athletes. Also, Dr Jeff Kindred, a sports medicine physician serves as one of the team physicians for GHS.
In order to participate in athletics for the upcoming school year, the paperwork may be completed beginning April 15, 2017. This includes obtaining a physical examination performed by a physician or other TSSAA approved healthcare providers. Deadline to complete this process is June 16, 2017. This way, once the TSSAA dead period is over, you do not hear that your son or daughter needs a physical exam and all paperwork by tomorrow or they cannot practice. This also allows the athletic trainer to view all physicals, complete paperwork check offs, contact coaches and parents with any issues, and make sure everything is complete prior to pre-season practices.
According to TSSAA, the need for a sports physical exam is when athlete begins practicing (or trying out) for the upcoming school year. Athletes are to have a current physical examination form on file in the parent school. Only physical exams dated after April 15, 2017 will be accepted and good for the entire 2017-2018 school year, according to the TSSAA guideline. I understand that you may have taken your child for a physical exam already in 2017, but following April 15 you will need to complete a new one for 2017-2018 school year.
This year we are using a secure online athlete portal system called Athletic Trainer System (ATS) to complete and collect all mandatory forms. The online ATS athlete portal can be accessed at https://atsusers.com/. Entering your athlete’s portal requires an athlete ID, password, and database. New or transfer athletes will need to create an account. The ID will be your child’s Student ID. Please DO NOT CHANGE THE ID to anything else. You may change the password, but please make sure your student will remember. If you have any issues with creating an account or logging in, or any other questions, please send an email to (firstname.lastname@example.org) stating the problem, athlete’s name, and best way to reach you. Please make sure all information is current. Under the “FORMS” tab, there are 8 REQUIRED forms to complete. For those who completed forms online last year a new feature is available. Open the form from last year, click the RED Verify button at the bottom, which verifies no information has changed. Then you can sign the form stating it is correct for the new year.

Again this year, Dr Kindred at Bodyguard Sports Medicine (615-328-3380) will be offering sports physical exams for $30. His office is located at Sumner Station, 225 Big Station Camp Blvd at the intersection with the bypass. A portion of the total proceeds from physicals he performs on Gallatin HS athletes will be giving back to the school’s Athletic Training program to purchase needed medical supplies and equipment to care for your athletes. Please consider Dr Kindred for your child’s physical, especially if you typically go to a walk in clinic.
